This plan is created for GGI patients who have achieved a successful recovery as a result of participation in our survivorship care plan.
Your plan provides ten hours of service over 9 months. Includes video calls as required. Assessing results, keeping you on track to maintain contact with your GP

This plan is created for GGI patients who have achieved a successful recovery as a result of participation in our survivorship care plan.

How to proceed
Patients need to apply to join a Survivorship Action Plan. The service will have been thoroughly discussed with you during a video call. It is essential to read our terms of service.
